Consent

BDO Svetovanje d.o.o. and BDO Revizija d.o.o., Cesta v Mestni log 1, 1000 Ljubljana (hereinafter: BDO), has a duty under the General Data Protection Regulation (EU 679/2016, hereinafter: the Regulation) to obtain the explicit consent of every natural person before collecting, processing, transferring or storing their personal data.
Exceptions to this are personal data whose processing is required by law or personal data that is necessary for the performance of a contractual relationship.
In accordance with the Regulation, BDO has the status of a Controller.
I understand that by signing this document, I give BDO my explicit consent for each individual purpose of collection, processing, transfer, and storage of my personal data.
I will do this by checking the appropriate box for each individual statement/line.
If I do not tick the individual statements, this means that I have not given my consent.
I agree that BDO may collect, process, transfer, and store my personal data.
I confirm that:

• I am aware of my rights regarding my personal data as defined in the Regulation (access, rectification, erasure, restriction of processing, transfer, objection), including the right to withdraw the above individual consents.
• I will exercise all my rights relating to personal data protection through the following contacts: (gdpr@bdo.si, 01 53 00 920). I may also use these contact details to request additional information regarding personal data protection.
• I am aware of the possibility of lodging a complaint with the Information Commissioner of the Republic of Slovenia (https://www.ip-rs.si/) in the event of a violation and/or restriction of my rights regarding the protection of my personal data.
• I understand that my individual refusal to give consent will in no way affect the business relationship between me and BDO.
• I am aware of the obligation to transfer my personal data to registrars and registers when this is necessary for the preparation and execution of the contractual relationship or when the collection, processing, transfer, and storage of personal data is required by law or in the public interest. Non-consent in such cases may result in BDO refusing to enter into a business relationship. I am aware that BDO will forward my personal data in accordance with my individual consent to its contractual partners who perform specific services on its behalf (in accordance with the Regulation: Processors) and who, in accordance with Articles 5, 6, 7, 9, 28, 29, and 32 of the Regulation and in accordance with the contract with BDO, are obliged to process and store only those personal data in accordance with good information security practices and for the purposes which are in accordance with my express consent in this statement.
• I am aware that each of the above individual consents is valid for 5 years from today's date.
